This is an auto draft authorization form for Evolution Functional Fitness Center. Registration is a one time fee of 25.00 and monthly memberships are 90.00. If you are participating in the mother/daughter tier program that I offer, the daughter registration is a one time fee of 25.00 and monthly memberships are 85.00 for the daughter.

The contracts are a SIX MONTH MINIMUM!! Early cancellation fees of 65.00 apply. You owe yourself at least six months to start this process.
